Veronika’s post: The shortage of primary physicians in the country has created a growth in the need for advanced nurses in different advanced nursing roles to provide better and cost-effective care, which supported the development of advanced nursing practice roles. However, different states used various APRN roles and scope of practice in education, certification, and licensure requirements. In 2008, the Consensus Model for APRN regulation was instigated to establish consistency in advanced nursing licensure accreditation and education to systematize the ARNP role, scope of practice, and requirements across the U.S. (Chamberlain University College of Nursing, 2022). Therefore, standardizing the APRN role requirements also contributes to the development of advanced nursing.
There are different barriers to confronting supportive sides for advanced nursing practice. For example, existing law regulations differ from state to state, whereas in restricted states advanced nurses have to practice with full collaborations with physicians. However, in some states, with Full practice, APRNs can practice independently. According to Kleinpell et al. (2022), a recent United Health report on PCP and NP scope of practice Laws recognized that if all states authorize nurse practitioners to practice independently, it will lead to a reduction of residents living with a shortage of primary care from 44 million to less than 30millions (70%reduction). The other barriers in advanced nursing practice within hospitals and other institutional organizations include a need for more understanding of the ARNP role, lack of professional recognition, and poor professional and administrational relationships (Kleinpell et al.,2020). Among the different nursing Domains identified by AACN, I would allocate competencies to my advanced practice roles, such as person-centered care and quality and safety, as my excel competencies. On the other hand, in the Personal, Professional, and Leadership Development domain, I have growth opportunities that I can obtain through a combination of practice guidelines, education, and practice as a personal growth as an Advanced nurse. Barriers to advanced nursing practice includes, roles that are not clearly defined and lack of standardization from state to state regarding practice requirements and role definition. Advanced practice nurses are required to understand political and practice implications on the state and national levels, and advancements in nursing policies. DeNisco, (2021, P. 239). Nurses must stay up to date with the current regulations to ensure that they are practicing safely and lawfully. Guidelines are different for direct and indirect care roles and may also vary based on the practice setting and other factors.
The American Association of Colleges of Nursing, (2021) states that essentials “create more consistency in graduate outcomes, influenced by the robustness of the learning experiences and demonstration of competencies”. Competencies vary based on advanced practice nursing roles and may interlock, be broader in covering multiple specialties or more specific to particular roles . In the family nurse practitioner program competencies related to direct care and clinical decision making are essential. Competencies are divided into domains to cover relevant information and topics in different aspects of advanced nursing practice. My experiences with working in multiple roles In healthcare has helped me to be able to see the big picture for the patients that I serve and provide effective person centered care. Scholarship for the nursing discipline is one of the domains that I am currently working towards growing In. This program is assisting me In that area by improving my skills in research and scholarship.

Some factors are a growing aging population and a decreasing workforce of primary care practitioners. Nurses were an obvious answer to the shortage as they are the largest group of healthcare professionals. Another contributing factor to the growth of advanced nursing practice is governmental support and policy changes (Torrens et al., 2023). As nurses have expanded their education and experience, the government has responded to pressures through several bills that have increased the scope of practice for Nurse Practitioners (NP) over the years (Kleinpell et al., 2023).
Numerous barriers exist to advanced nurse practice are, unfortunately, physicians. Physicians have attempted to block the advancement of nurse practitioners’ having more authority in practice. The transition to practice in some states has also been a barrier for NPs. Transition to practice means NPs have to complete a certain number of hours or years under the mentorship of an experienced clinician before they can work with full authority. These regulations vary by state but hold NPs back from applying their training and education to the fullest potential (Kleinpell et al., 2023).
There are several core competencies for NPs, such as scientific foundation, leadership, quality, policy, ethics, and independent practice. A competency I excel in is quality care. By using research and current data, I think it is essential that we apply evidence-based data to the patient’s care. Evaluating the data and applying the evidence to improve clinical practice is important. Evaluating how processes and systems can be modified based on research is vital to providing safer, more efficient, cost-effective quality care. As far as a domain that would be a growth opportunity for myself would be policy and healthcare guidelines. I look forward to learning more about the policies that affect our roles and, ultimately, our patients. I am interested in advocating for equity and ethical policies and understanding what drives critical decisions in policymaking (Yaqoob et al., 2021).
